Ok, so I got it and tested it. If your health potion is on cooldown and then you trigger Potion Surge, you're still gonna have to wait for it to come off cooldown and then you can instantly drink 2 in a row. Also it seems that first this perk applies -100% potion cooldown, but then +25% is added from Endless Thirst. I don't have refrershing toast on my ET, so I have a ~5.5 seconds cooldown between 2 pots.
Here are approximate healing numbers at around 20k health:
6200 ET+divine+10% from SnS (doesn't work on both weapons)
5800 ET+divine
5400 ET, no divine
4700 divine, no ET
4200 no divine, no ET

What happens when the "Survive" perk procs but your healing potion is on cooldown? Does it reset the cooldown or do you have to wait for the cooldown and then you can drink 2 potions in a row?

So its like Purify and Stamina Recovery, both Amulet perks, on a chest piece with HP pot recovery. Inb4 meta.
