Dropped after 41 kills. Respawn time is 2 minutes. Between kills you can go to the chapel nearby to kill Ivan the Scourge for the Jewelcrafter's tunic, who has a similar respawn time.

I was doing Myrkard everyday for a long time now wiht a group of people and no1 got this hat. I doubt it is from chests there... It might be from bosses (they rarely drop anything for us) but chests would drop this by now.

I can confirm this item does exist, somebody was trying to sell one on Global tonight and they linked it. They claimed it was only dropped from Captain Thorpe's Room in Myrkgard Cathedral, I can't confirm if that is true or not but all the "dropped by" do list that location.
